First Visit...

In preparation for your first visit to Christian Counseling Center, the Front Office Staff will direct you to the appropriate forms to download and bring with you to your first appointment. Having these forms in advance will reduce your registration time so that when you go into session with your therapist the paperwork will already have been completed. A reminder ... don't forget your INSURANCE CARD.

Helpful hints to prepare for a child/adolescent appointment...

The Child - Adolescent Intake/History Form should be filled out for children through Grade 12.

If applicable, please bring the additional information to your first appointment...

• previous psychological evaluations
• copies of divorce and/or custody documents
• prepare separate releases for Primary Care Physician and School Teacher

Helpful hints to prepare for an adult appointment...

• Print and complete the Adult Client Information Sheet (Welcome Sheet)
• Please read the Consent For Services and Fee Agreement prior to your arrival , the Front Office will have a copy for you to sign at check in.
