If you have made an appointment you will be directed to fill out the following forms. In completing the forms you have two options:

• DOWNLOAD and Print
• SUBMIT ON LINE using E-MAIL

1. To DOWNLOAD just "click" on the form that you need, print it, complete the form and bring it in with you when you come for your appointment.
2. To SUBMIT ON LINE using EMAIL open the form that you need by clicking on it, fill out the form, and when you have completed the form, "click" on SUBMIT FORM. Sometimes you will be presented with an option for how you want to send it, please choose "Send Copy" only. It will now attach the completed document to an email. Please input This e-mail address is being protected from spambots. You need JavaScript enabled to view it in the to field and press send. To help us further you can also add the date of your appointment to the subject line of the email.

ADULT:

• Client Information Sheet - Submit or Copy
  

The Consent and Authorization forms can only be DOWNLOADED AND COPIED.
